Three of the most commonly used neonatal assessments in the delivery room and nursery:

APGAR Score: Developed in 1952 by Virginia Apgar, this five-sign scoring system assesses newborn status at 1 and 5 minutes after birth. Each sign is scored 0-2 for a total of 0-10. The five signs are: Activity (muscle tone), Pulse (heart rate), Grimace (reflex response), Appearance (skin color), and Respiration. Scores 7-10 are reassuring. Scores 4-6 indicate moderate concern — resuscitation assistance is usually provided. Scores 0-3 indicate severe concern requiring immediate intervention.

The 1-minute score reflects intrauterine conditions and the transition to extrauterine life. The 5-minute score is more predictive of longer-term outcomes. A score below 7 at 5 minutes is associated with increased risk of hypoxic-ischemic encephalopathy.

Gestational Age from LMP: The most common way to date a pregnancy is Naegele’s rule: due date = LMP + 9 months + 7 days (or equivalently, LMP + 280 days). Gestational age is counted from the first day of the last menstrual period. A full-term pregnancy is 37-42 weeks.

Birth Weight Percentile: Birth weight is classified by gestational age using Fenton growth charts (2013 revision). LGA (large for gestational age) is above the 90th percentile. SGA (small for gestational age) is below the 10th percentile. AGA (appropriate for gestational age) is 10th to 90th percentile. This tool uses simplified reference ranges based on published median values.
