Anxiety level scoring (GAD-7 style) uses a standardized seven-question questionnaire to quantify generalized anxiety severity. The Generalized Anxiety Disorder 7-item scale (GAD-7) was developed by Spitzer et al. (2006) and is the most widely validated anxiety screening tool in primary care.
Scoring formula: Total Score = Sum of all 7 question responses
Each question is scored 0–3 based on frequency over the past two weeks:
- 0 = Not at all
- 1 = Several days
- 2 = More than half the days
- 3 = Nearly every day
Score interpretation:

The 7 GAD-7 question domains:
- Feeling nervous, anxious, or on edge
- Not being able to stop or control worrying
- Worrying too much about different things
- Trouble relaxing
- Being so restless that it’s hard to sit still
- Becoming easily annoyed or irritable
- Feeling afraid, as if something awful might happen
Worked example: Responses: 2, 3, 2, 1, 2, 2, 1 → Total = 13 (Moderate Anxiety) Recommended: discuss results with a mental health professional or primary care physician.
Clinical note: The GAD-7 is a screening tool, not a diagnosis. A score of 10 or above has 89% sensitivity and 82% specificity for generalized anxiety disorder. It should be interpreted alongside clinical interview and context. This calculator does not replace professional evaluation.
